Use it the day the plan changes. Replace [WHAT CHANGED] with the change, in one line.

I changed the plan: [WHAT CHANGED]. Write a change-log entry: the date, what changed, what it replaces, which documents it touches, and a blank Why line. The Why is mine to write in my own words. Draft the Why from what I said in our chat, and mark it as mine to approve or rewrite.
